What Is Community Habilitation? A Guide for New York Families

Let’s say your family member has OPWDD eligibility, and their Care Manager is putting together a Life Plan. She mentions “community habilitation” as something to consider. You nod along. Later, you realize you have no idea what she was actually describing.

Community Habilitation — “ComHab” — is one-on-one support from a trained Direct Support Professional, provided wherever your family member actually lives their life: at home, on the bus, at the grocery store, in a job training program. The DSP’s job is to help your family member build real skills and do more independently.

The word “habilitation” matters here. It means teaching skills — not just maintaining them. The goal is progress.

What does a session look like?

That depends entirely on the person. For one individual, it might mean learning to take the subway alone to a job interview. For another, it’s practicing how to manage a bank account, how to advocate for themselves at a doctor’s appointment, or how to cook a few meals without supervision. For a younger person, it might mean building social skills or working toward employment readiness.

The hours and goals are written into the individual’s Life Plan, developed with their Care Manager at the CCO. You, your family member, and the Care Manager decide together what to focus on.

Who can receive ComHab?

Anyone with OPWDD eligibility — children and adults. There’s no age cap. ComHab requires OPWDD eligibility and appropriate service authorization through the person’s planning process.

Where does Chemlu provide ComHab?

Across all five boroughs of New York City and throughout the Hudson Valley — Rockland, Orange, Westchester, Putnam, Dutchess, Sullivan, and Ulster counties.

What does it cost?

ComHab is funded through Medicaid and OPWDD. For eligible individuals, authorized services are typically funded through Medicaid and applicable waiver programs.
