Social Care Network Screenings & Navigation
Good health starts with the basics: food on the table, a stable place to live, a way to get to appointments. Through New York's Social Care Network (SCN), Chemlu screens eligible Medicaid members and helps connect them with food, housing, transportation, and other social-care resources when available — at no cost for the screening, whether or not you receive any other Chemlu service.
How It Works
1. A short screening
A Chemlu team member asks a few plain-language questions about your household's needs — food, housing, transportation, safety. It takes minutes, in your language, by phone or in person.
2. A real connection
We match your needs to the right resources — food programs, housing assistance, transportation services — and make the referral for you.
3. Follow-through
Navigation means we follow up and work with you toward a real connection — not just handing you a phone number. That's the whole point of the SCN.
Where We Provide SCN Services
The Bronx
Chemlu provides SCN screenings and navigation in the Bronx as a contracted partner of SOMOS.
The Hudson Valley
We serve Westchester, Rockland, Orange, Putnam, Dutchess, Sullivan, and Ulster counties as a contracted partner of Hudson Valley Care Coalition (HVCC).
Any Medicaid member in these regions may receive an HRSN screening at no cost. Eligibility for navigation or enhanced services depends on Medicaid coverage type, identified need, region, and program rules.
For Care Managers, Clinics & Community Partners
Chemlu accepts SCN referrals in both regions. We complete HRSN screenings, provide navigation with documented follow-through, and coordinate closely with care teams — including for individuals with I/DD, where our staff bring twenty years of experience. Families we serve through OPWDD and Children's HCBS can also request a screening from any Chemlu staff member — a need that comes up in conversation shouldn't get lost between agencies.
